Good Samaritan Clinic is seeking a compassionate and dedicated Diabetes Educator to help improve the health and well-being of uninsured individuals living with diabetes in Burke County and surrounding communities. This position plays a vital role in empowering patients through education, support, and access to resources that promote healthy lifestyles and improved diabetes outcomes.
As a member of our interdisciplinary healthcare team, the Diabetes Educator will provide individualized and group diabetes self-management education, support grant-funded diabetes initiatives, and work closely with patients facing barriers to care, including farmworkers and other vulnerable populations.

As a member of our interdisciplinary healthcare team, the Diabetes Educator will provide individualized and group diabetes self-management education, support grant-funded diabetes initiatives, and work closely with patients facing barriers to care, including farmworkers and other vulnerable populations.
Primary Responsibilities
- Provide diabetes self-management education and support to patients with prediabetes, Type 1 diabetes, and Type 2 diabetes.
- Educate patients on blood glucose monitoring, medication management, insulin administration, nutrition, exercise, and complication prevention.
- Develop individualized care plans and self-management goals.
- Manage a diabetes education program funded through grants.
- Track and report grant outcomes, including A1C reduction, patient engagement, attendance, and other quality measures.
- Coordinate diabetes supply assistance programs, including glucometers, test strips, continuous glucose monitors (CGMs), and insulin assistance.
- Collaborate with providers, nurses, pharmacists, behavioral health staff, and community partners to support patient care.
- Document patient encounters and maintain accurate records in the electronic medical record system.
- Conduct community outreach and educational programs to increase awareness and prevention of diabetes.
- Support farmworkers and other vulnerable populations through culturally appropriate diabetes education.
- Assist patients in accessing medication assistance programs, community resources, and social support services.
- Participate in quality improvement activities and program evaluation.
Required:
- Bachelor's degree in Nursing, Dietetics, Health Education, Social Work, Public Health, or a related healthcare field.
- Experience in diabetes education, chronic disease management, primary care, or community health.
- Excellent communication and patient education skills.
- Certified Diabetes Care and Education Specialist (CDCES) credential or willingness to obtain.
- Experience working with uninsured, underserved, rural, or migrant populations.
- Bilingual English/Spanish preferred.
- Experience with grant-funded programs and outcome reporting.
- Strong understanding of diabetes management and current clinical guidelines.
- Ability to educate and motivate patients from diverse backgrounds.
- Experience with motivational interviewing and behavior change strategies.
- Strong organizational, documentation, and data-tracking skills.
- Commitment to compassionate, patient-centered care.

"What an excellent facility. The whole staff at GSC are amazing. Their mission is to help what is
considered “vulnerable” populations who need medical care in the local community. They opened it
looks like in 1995 and provide more than $5 Million in free medical care annually. They are
interdenominational, have medical care, dental professionals, Vision Care, Mental Health Counseling,
Farm Worker Outreach, Confidential HIV Testing, an in-house Lab Tech and a Dedicated Pharmacy
among other services. But overall, they are VERY kind people who really care about helping others."
